The MediFit Clinic collects and stores the information you submit on The MediFit Clinic’s website, and information we receive through our interactions with you.
Required Information : To schedule a consultation with The MediFit Clinic, you must provide your name, phone number, and email address.

- Patients of The MediFit Clinic who have proven successful on their weight loss program may be invited to partake in our Testimonial Program. Patients that participate in our Testimonial Program are not compensated for their involvement. Testimonials include a before and after picture, name, total weight loss and a personal statement. Testimonials are distributed to print media and electronic media, including internet, television and radio.
- We disclose information that we in good faith believe is appropriate to cooperate in investigations of fraud or other illegal activity.
- We disclose information in response to a subpoena, warrant, court order, levy, attachment, order of a court-appointed receiver or other comparable legal process, including subpoenas from private parties in a civil action.
- We disclose information to your agent or legal representative (such as the holder of a power of attorney that you grant, or a guardian appointed for you).
